NEW PATIENTS

We are currently accepting registrations from those who reside within Leicester, Leicestershire, Rutland and the District of Harborough

To register at Spectrum Health, each new registrant needs to complete a GMS1 form. The quickest and easiest way to do this is to complete an Interactive Registration Form (IRF) via the button above.

Alternatively, you can download the registration forms (links are available at the bottom of this page) and upon completion these forms will need returning to the practice by emailing them to spectrum.health@nhs.net or by dropping them back to the practice.

You can also collect registration packs from the practice.

If you are on regular medication, it may be useful to send us a photo of your medication showing your details and the preparation, or bring the packaging to the practice in person.

Medical treatment is available from the date of registration. Please contact reception for further information.

REGISTRATION FORMS

GMS1 Registration Form
Adult Registration Form
Child Registration Form

Please download and complete ONE GMS1 form and ONE Registration form for each new registrant.

INCOMPLETE FORMS – Please check every question has been answered.

If you have NEVER had a UK GP before, we need the date you entered the UK.

If you get a message to call us about your registration it is usually because the NHS has a different address than the one you gave us, please call us urgently.

CATCHMENT AREA

From January 5 2015, all GP practices in England are free to register new patients who live outside their practice boundary area.

Please note: These new arrangements are voluntary for GP practices. If the practice has no capacity at the time, or feels it is not clinically appropriate or practical for you to be registered so far away from home, then we maintain the right to refuse. We shall explain our reason for refusing your registration to you.

In view of the above, we will register patients who reside with Leicester, Leicestershire, Rutland and the District of Harborough; however only those within our inner practice boundary (Oadby Town) will be entitled to domiciliary care such as GP home visits.

Distance from the practice is not an issue if you are willing to travel to the practice and / or happy to receive video, telephone or internet consultations.
